Sliding Fee Discount Program

East Grand Health Center offers a Sliding Fee Discount Program in order for patients to be seen for their medical necessities regardless of their ability to pay. The sliding fee schedule is based on family size and income; it changes annually and is set by the Department of Health and Human Services.

Click here to download East Grand Health Center's Sliding Fee application. To apply, you may fill out the application and return it with your most recent Federal Income Tax Return or TWO of the following:

  • Three months recent bank statements

  • Social Security Benefit Statement (if applicable)

  • Four weeks of recent paycheck stubs

  • Unemployment statement

  • TANF statement
