Making an appointment...
East Grand Health Center wants to be your medical home! In order to do so, please fill out a Medical Records Release form to transfer care to EGHC.

Preparing for your first visit...
If this is your first visit to East Grand Health Center, you should plan to arrive 15 minutes early. This will allow time for you to complete the patient registration form. If you have health insurance, please remember to bring a copy of your insurance card with you as well as your co-payment and/or coinsurance.
Want to inform your new provider of your medical history prior to your first visit? Print and fill out our New Patient Questionnaire to allow your provider the opportunity to get to know you better.
