About EGHC
East Grand Health Center was founded in 1965 and was the first rural health center in the state of Maine. Currently, it is the local health center for 15 Maine towns and townships as well as the summer residents of East Grand Lake.
The original East Grand Health Center was located in one room of the Danforth Town Office. It was open one day a week with one medical provider and one public health nurse, serving as a nonprofit ambulatory health care center. In 1981, the current health center at the 201 Houlton Road location was built after local citizens saw the need for a more permanent facility.
We are fortunate to have an extremely dedicated Board of Directors, several of whom have remained on the Board since the health center's inception. They all reside in the community; many are health center patients. Their insight and judgment has ensured that the health center continues to grow to meet the needs of the local community.

Our Mission: The mission of the East Grand Health Center is to promote the health and wellbeing of the people in a medically underserved remote region.
Our Vision: The East Grand Health Center is a federally qualified community health center. It was organized to develop and maintain a health care delivery system of the highest quality for all people regardless of social or economic circumstances, in a confidential, respectful, and humane manner. The vision of the East Grand Health Center is to provide a medical home for the residents of the area. We strive to provide high quality, cost-effective primary health care services to all who need them. We collaborate with all other health care service providers in the area in an attempt to provide the most comprehensive services possible in order to best serve the needs of our community.
Meet Your Care Team
Medical Director
Dr. Mark Kaplan, DO
Dr. Kaplan received a Bachelor of Science degree from Boston University, a doctorate in osteopathic medicine from Kansas City College of Health Sciences, did an internship and residency at Botsford General Hospital in Farmington, Michigan and a pulmonary fellowship at Queens Medical Center in New York. He has specialized in critical care and emergency medicine since 1988.
Dr. Kaplan specializes in urgent and primary health care. He is certified by the American Osteopathic Board in internal medicine, and board eligible in emergency and pulmonary medicine. Dr. Kaplan has served as the Medical Director of East Grand Health Center for the past several years and is a valued member of the community.

Matt Cowan
Matt is a Physician Assistant and has been practicing family and internal medicine for over 20 years. Matt is also certified for DOT physical exams. He is a longtime resident of the area, currently residing in Houlton with his wife, Molly.
LCSW
Terese Line
The addition of Terese (Terrie) Line is an important part of EGHC's mission to expand mental health services to our community. Terrie supports patients who struggle with anxiety, depression, substance abuse, domestic/physical abuse, Bullying, grief/loss, or parenting/situational life issues. During each session, she works with the patient to gain coping skills and set goals that are achievable for the patient. Terrie works with patients of all ages starting at the age of 5. All services are conducted through telehealth and available five days per week.
FNP
Lyndsay McIver
Lyndsay is a Family Nurse Practitioner that specializes in family medicine and treats patients of all ages. Lyndsay joined East Grand Health Center in 2019 as a Registered Nurse and then graduated with her Master's in Nursing in 2020 from Husson University. Lyndsay is a resident of Lambert Lake with her husband and sons.

Dr. Jared Kohlbacher
Dr. Jared Kohlbacher sees patients for podiatry services and has been a member of the East Grand Health Center team since August 2017. Dr. Kohlbacher is a podiatry specialist originally from Oklahoma. He graduated from New York College of Podiatric Medicine in 2011 and completed his residency in 2014 at St. Barnabas Medical Center in New Jersey. Dr. Kohlbacher currently provides surgical podiatry at The Aroostook Medical Center. He and his wife, Raina, and their three daughters reside in Mapleton.
Dr. Kohlbacher is at the East Grand Health Center on a monthly basis.

Tiffany Leach
Tiffany is a Physician Assistant that joined East Grand Health Center March 2025. She has been practicing family and internal medicine for over 12 years after graduating from the University of New England. Tiffany currently resides in Hodgdon area with her husband and daughters.
PNP
Dayna Lincoln
Dayna sees pediatric patients for primary care services and has been a member of the East Grand Health Center team since March 2024. Dayna specializes in pediatric patients from birth to 21 years of age. She graduated from Liberty University in 2012 with her Bachelor's and completed her Master's Degree in 2021 at Maryville University in Missouri. She and her husband, Emery, and their two children reside in Hodgdon.
